Here's the translation of the record -

Before the undersigned civil registrar appeared today, recognized on the basis of a membership card, the worker Otto Ladewig, residing in Berlin-Malchow, Ullerweg 8, and declared that the master shoemaker Hermann Ladewig, 82 years old, residing in Berlin-Charlottenburg, Sophie Charlotte Strasse 115, born in Königsberg in Neumark, married to Auguste nee Polz[in], residing in Berlin-Malchow, Ullerweg 8, died in Berlin-Charlottenburg, Sophie Charlotte Strasse 115 on 18 May 1938, in the afternoon at 9:15. The informant declared that he has personal knowledge of the death.
