Birth Record No. 3930, Lódź, Roman Catholic Parish of the Blessed Virgin Mary, of Michalina Galasiak, born in Lódź on the 28th of October 1908, christened on the 5th of November 1908, daughter of Jan Galasiak, a day laborer from Lódź, 32 years old, and of his wife, Antonina, born Kujawa, 23 years old.

Witnesses: Jan Rudnicki, and Jozef Miasko(?), day laborers of legal age from Lódź.

Godparents: Jan Rudnicki and Walentyna Zaionc.
