Baptism and Birth Record No. 88, Piotrkow Kujawski, Roman Catholic, of Elzbieta Kubiak, born in Wasewo on the 2nd November 1828, christened on the 9th November 1828, son of Tomasz Kubiak, a farmer in Wasewo, 33 years old, and of his wife, Antonina, born Smolarska, 30 years old.

Witnesses: Szymon Marszałkowski, a farmer, 39 years old, and Szymon Kornacki, a farmer, 38 years old, both from Wasewo.

Godparents: Paweł Kamiński and Elzbieta Marszałkoska.
