question about name of hometown

When requesting civil records or church records do you use the ancient name of the town or the modern name? If it is the modern name, what would it be for Weida, Neustadt, Sachsen-Weimar-Eisenach?

But the hierarchy is now: Weida - Landkreis (district): Greiz - Bundesland (state): Thuringia/Thüringen
