I would appreciate a translation of the attached document. I can get the gist of it - Charles Tolkowsky, son of Abraham Tolkowsky (a merchant?) and Mina Goldberg from Antwerp and a student (?), died in Wiesbaden in 1908 - but I haven't been able to decipher much of the handwriting.

Before the undersigned civil registrar appear today, personally recognized on the basis of an identity card, the merchant "Schilem" called Salomon Tolkowsky, residing in Antwerp, 17 Avenue Cogels, and indicated that the student Charles Tolkowsky, 16 years old, Jewish religion, living in Antwerp with the informant, born at Antwerp, son of the informant and his wife Minna, née Goldberg, died at Wiesbaden, Schwalbacherstraße 44, on the 18th of May 1908 at 7:00 pm.
Read aloud, approved, and signed: Salomon Tolkowsky.
The Civil Registrar, as a representative: Kray.
Certified to be in agreement with the Main Register, Wiesbaden, on the 19th of May 1908.
