Translation help for Herren Sulzbach baptism record

Please help me translate this record. I think it's a baptismal record, but after the baby's name, Anna Elisabeta, nothing seems right to me and I'm struggling with a translation. I think the child died either the day after birth or maybe the same day. I think it says something about "poor mother". I'd appreciate it if someone could help with this translation.

Some of the record is lost in the center binding too!

Hi,

'On Wednesday, the 8th of March, Johann – Simon von Sultzbach and Maria ?Försterlein? have baptised their daughter Anna Elisabeth. [The child] died that same day shortly before its poor mother. We will – at this place. [His?] forefathers were goths from Slutzbach, Hamberg of – – – recorded –.'
