What an interesting document! Where did you find it?
I skimmed through it a bit (can give you more of a complete translation if needed), but it discusses nobility of the Kozlowsky family in Smolensk.
It references a nobility book (you can read more about those on this link https://ru.wikipedia.org/wiki/%D0%94%D0%B2%D0%BE%D1%80%D1%8F%D0%BD%D1%81%D0%BA%D0%B0%D1%8F_%D1%80%D0%BE%D0%B4%D0%BE%D1%81%D0%BB%D0%BE%D0%B2%D0%BD%D0%B0%D1%8F_%D0%BA%D0%BD%D0%B8%D0%B3%D0%B0 The webpage is in Russian, but you can use google translate to help you read it), part 6, and some specific pages (39, 61, 62, 263, 275). There is some other information there, but I'm not sure what the abbreviations stand for.

The older gentleman I am trying to help out is related to these fellows in the document. He has some documents, but others where lost over the years. Thank you for these interesting clues, I'll start following the rabbit holes..
