I have been trying to locate her birth cert for over 6 months having contacted the commune in Sciacca 3 times (1 time by snail mail, 2 times via email) and I have not received a single response. Looking for some assistance as I just found this info with Todaro a few days ago. Could her birth name have been Todaro, but it was changed in the US?

It looks like the database you refer to have some issues of data consistency.
Those are infact genealogy tree "imported" by FS and never updated.
I tried to look for the birth certificate of the person who is reported there as the father of Antonia Todaro/Butera: Domenico /Todaro <Butera>/ born on 28 AUG 1839 at Sciacca, Agrigento Italy.
There is in fact a birth act for a similar person in the Antenati website:
HOWEVER: his name is Domenico Todaro (nobody by the name of Butera appears in the record) , his father is named Giuseppe (not Alberto), while mother has same name as in the database (Girolama Cataldo of 16 years).
I also did a quick check to see if any Domenico Butera was born on that year - despite three newborn Butera, none was named Domenico.
In FamilySearch there also seems to be an Antonia Todaro LRHX-K7R, daughter of a Domenico son of an Alberto Todaro (AKA Alberto Todaro Butera), who, BTW, also have some duplicated parents....
All in all, I think there are two families here (Todaro and Butera) that have been mistakenly merged.
My suggestion would be to start from available records of your Antonia Butera, "digging your way back" across all her life events and see where you land...

HOWEVER,
I found this birth act of a "Maria Todaro Butera" dated June 27, 1899
It's the birth registration of a Maria Todaro Butera, daughter of Domenico Todaro Butera and Ignazia Santangelo (both match your Antonia's parents).
On the side there are two VERY interesting notes:
1- on sept 16, 1890, the last name of the father gets rectified in "Todaro" and NOT "Todaro Butera"
2 - on jan 8, 1927 (?) Maria Todaro (Butera) married Pietro Savona of Sciacca.
In conclusion, this Maria is not your Antonia, but her birth act provides an exlaination of the Todaro Butera issue: apparently just a clerical mistake!

Well, after all it might not have been a clerk's mistake!
When Antonia was born, on March 16, 1886, she was registered as Antonia Butera Todaro, parents Domenico Butera Todaro, mother Igazia Santangelo.
For some reason, Domenico later changed mind about his last name and had it shortened to "Todaro", but only changed the last name of Maria (in 1890) , and NOT that of Antonia.
As a matter of fact, when Antonia arrived to Ellis Island on July 9, 1902, she was registered as "Todaro Butera Antonia".
Side comment: she was actually due to travel to US a month earlier, but for whatever reason she didn't make it.
In fact her name (Butera Antonia Tadaro) is crossed out in the ship journal of June 2, 1902!

It looks like in later years Antonia chose to be only "Butera", hence her name at marriage and death.
PS: maybe also due to the above, the FS trees related to Todaro and Todaro-Butera are quite messy, with many duplicated individuals and wrong links. Some of the descendants may need to sort it out!

OH MY GOODNESS!! This is what I have been searching for. Thank you, Paolo! I have no idea how you found this document, you are an angel. Would you have any idea what I should reference to request this document from Sciacca for them to build her birth certificate? Sorry, I can't read the cursive Italian.
I will do my best to sort it out with the information I have and correct what I can on FS.

Sicily created a birth certificate like this below for my great grandfather. The word "build" was used which is why I used it. Thank you again!

This looks like a certified copy of a birth certificate where thy have typed up the information and certfied with a notary stamp. This is used, for example, if you are trying to get an Italian citizenship - you have to have a certified birth certificate. Otherwise, the copy from the register book works just fine. You have to write to the comune to get a certified certificate.

if you need an official certificate to request itaian citizenship, you have to provide to the Comune di Sciacca all key info from the act of birth of Antonia Butera Todaro , like: location (Sciacca), birth date (14 marzo 1886), parents (Domenico Butera Todaro and Ignazia Santangelo).
You should address the request to :

I suggest you explain you are a descendant, attaching also a copy of your ID (passport or other).
Remark for when you will need to present the request for citizenship to the Italian Consulate:
Due to the fact that Antonia shortened her family name from Todaro Butera to Butera, you might be requested to proof she is the same person who married Paolo Arena in 1905.
A copy of the original marriage act by the Norristown Italian Parish (which includes her birth date) may help you.
Anyhow, here is a 1910 transcription of the marriage act that might eventually be enough.

Thank you! I have already applied for my dual citizenship via Chicago which is the only consulate in the states that requires non-line certified documents. I have submitted Antonia & Paolo's marriage certificate, as well, but it does not include her birthdate unfortunately. I greatly appreciate your help and I will request her certificate from Sciacca today via the email you suggested. Grazie mille!

Hi Paolo. Do you know if the commune will respond to my request? I immediately sent them an email with all of the information a month ago (it was actually a third request), but have not had a response to the email. I know not to expect a paper copy for a long time, but I'm wondering if they acknowledge requests? This is the very last piece I need for my dual citizenship application. I just want to be sure it was received. Thank you!

Unfortunately it does depend very much on how the municipality is organized.
Last week monday I got a document from Vasto tht I did requestthe saturday before - really surprising service!
On the opposite, Tuesday I finally received a death record I requested to another municipality almost 3 (yes, THREE!) year ago, and solicited several times!
