The original form was for a marriage record, but the marriage record info was crossed out and the death record headers were handwritten in. I don't really read German or Latin, and I think it might be a mix of the two.

My best guess on what I can read is:

Tag das Begräbnisse [Day of Burial]: den 15 ?? I think this might be the burial date, which based on the context might be the 15th of May?

Art und Ort das ???????: I have no idea what this says. I think the column might be about where he was living? Or maybe where he was buried?

Namen das Verstorbene [Names of deceased]: ???? Johan Carl Gottfried Ecke

????????????: ????????????????? I think this might have to do with his occupation?

Alter [Age]: 42 years

Ursache das Tode [Cause of death]: Blutsturze [Hemorraging] (I'm not entirely sure about the cause of death)

Translation: The death notice listed under no. 25 belongs under no. 21

I am not sure what it refers to.
