Birth Record No. 129, Kampinos Parish, Priest acting as Officer of Civil Records Office, Powiat Sochaczew, Department Warszawa, of Roza Wojtysiak born in village Zalasek on the 27th December 1811, birth reported on the 28th December 1811, daughter of a maiden, Dorota Woytysiakowna (daughter of Wojtysiak), 25 years old, who has lived in village Zalasek for the last 2 months. Birth reported by Marcin Polec, 52 years old. Witnesses: Kazimierz Polec, 56 years old, a farmer from Kampinos, and Ignacy Michalowski, 54 years old, a farmer from the same village.
Woytysiak = diminutive of Woyta, like an adjective, e.g. Smith, Smithy, of Smith.
