The specific church isn't listed; it just says she was baptized at "the church of this comune", which is almost certainly the chiesa matrice - Parrocchia SS. Martiri e Medici Cosma e Damiano

The handwriting is not too easy to read unfortunately but I'll try my best since I am used to reading these documents by now:
Birth record (atto di nascita) number 110
The year 1853, the 18th June at 14:00, in front of us, the mayor of Alberobello (district of Altamura, province of Terra di Bari) Giuseppe Martellotti (I have checked the name of the mayor in the other records as well because it is hard to read it, it could be the same surname of Margarita, so maybe Martellotta instead), arrived Antonio Vito Martellotta (it might also be Martellotti like the surname of the mayor as well, but to me it looks more like Martellotta here), 33 years old, farmer (profession), resident in this town/village (I believe it says "in questo comune") who presented/introduced and showed us a female baby whom we visibly recognized and he declared that she was born from his legitimate wife Angela di Caro, 27 years old (I am not 100% sure about the spelling surname, but you might be familiar with it already and I have checked the spelling in the index as well, at the end of the collection here at page 214, number 104 on the right page: https://www.antenati.san.beniculturali.it/ark:/12657/an_ua243672/wQEBB6Y). The wife is 27 years old, profession: farmer (as above), address: same of her husband, and the child was born on the 17th June of the current year (1853) at 12 (twelve o'clock) in the house where the parents live.
The document continues in the following page, page 117 on the left but first there is the baptism record on the right side of the same page that I quickly summarize here:
The day Margarita Addolorata Martellotta (again, it could be spelt Martellotti instead, the last letter is not easy to read) received the sacrament of the baptism was the 18th June 1853 by the priest of the church of Saint C...(I can't read the name of the church unfortunately)
Continuining now from page 117 on the left: the father (Antonio Vito) has declared as well that he has given to his daughter the name "Margarita Addolorata"
The following part is about the witnesses but I will include it briefly
This was done in the presence of Giuseppe Porta, 30 years old, and of Pietro Orazio Ci..la (I can't read the surname, sorry), 43 years old, the profession for both of them is "taglialegna" (woodcutter/lumberjack) and they are both resident in that same "comune" (town/village of Alberobello) and the two witnesses were chosen by Antonio Vito Martellotta
It ends with the signature of the mayor and the names of Antonio and the two witnesses (they can't write and therefore can't sign the paper)

Thank you, Cousin Vinny! for the information on the church Parrocchia SS. Martiri e Medici Cosma e Damiano. This is the church that my grandfather was given to by his mother Margarita Addolorata Martellotta. He was born illegitimate and taken care of by a foundling home. I would like to contact the church for more information on my grandfather, how do I do that? I have his birth record, death, and his marriage, but none states the birth father's name only the mother's. I figure the church must have other records kept in the church, I would like to find out more information. My grandfather Filippo Martellotta was born in Alberobello on 25 May 1889 and married Rosa Contegiacomo in 1912 Alberobello. My grandfather was given the surname of Negri, but his mother gave her surname just before he was married. As far as I know, the mother was not married. I have been looking and looking for my grandfather's biological father. I have a feeling there is more information held in the church. Would you be able to help me write to the church and ask about any information they have on my grandfather? PLEASE!!
