Needing translation help, please

I'm helping my friend and need help reading the town of birth of her grandmother, Helene Grau, KCR1-QFD, and her mother's maiden name. Her records in familysearch show the mother's name as Helene Butchereit.

Please see entry # 11 for Helene Grau, birthplace (?), age 14, born 26 January 1895. I'm not sure what the next column stands for nor can I read the name of the church in the next column. Her parents are Edward and Helene (born ___________?)
