This is the last known address for my family in Ukraine. Can someone please translate? Is the first line a surname? My relative married but I don't know her married name. Thank you so much.

The last line looks to me like Корусь Вале, but I'm not sure what this might mean.

Korus Valya (Корусь Валя) is the name of the addressee of the note, Korus is the surname, Valya is either Valentina or Valentin (this name is given to both boys and girls)
