Midwife: what is her maiden name and married name? Also, what does it mean by reformed?

2nd witness: his occupation is a schoolteacher but what kind. Again, with the reformed.

The name of the midwife:

Anna Barbara geborene Hardtin, verwittwete Böhlin = Anna Barbara née Hardt, widowed Böhl

"reformed" may refer to the religious community, the Reformed Church (https://en.wikipedia.org/wiki/Continental_Reformed_church), a branch of the protestant church. I've seen this in connection with teachers, not with midwives so far.
