Yes a headstone can be a primary source.
No cemeteries are not required to meet standards for accuracy.
I am unsure on your 3rd question. I THINK it would be one of your best sources in that time period. One year off is not uncommon on a persons headstone. I have even seen where the (place) that the cemetery is in records and the headstone differ.
Such as the city where I live keeps records on our cemetery and they do not always match the information on the headstone.
