Translated from Latin:

Marriage Record No. 3, Jawornik Polski Parish, dated 13th February 1849, of Michael (Michał in Polish) Piątek, House No. 38, 42 years old, a widower, farmer, and of Magdalena Łysiak, House No. 19, a maiden, 20 years old, daughter of Jacobus (Jakub) Łysiak and of Magdalena, born Jamrozik. Witnesses: Valentinus (Walenty) Gołąb and Michael (Michał) Kawaliło. Andreas (Andrzej) Jamrozik and Matheus (Mateusz) Łątka.

Written in Polish at the bottom of this Marriage Record: As the father of my underage daughter, Magdalena Łysiak, I give permission for her marriage union with Michał Piątek. Jakub Łysiak, father of the bride. Witnesses the same as above.

There is also a note in Latin of Wl. Blotnicki poss. re Marriage Bann, dated 9th Feb. 1849 in Szklany, No. 74.
