Transcription and translation request for 3rd ggf birth record

I wanted to get a transcription and translation of this birth record for Samuel Werner, my 3rd ggf, from the 1814 Momehnen, East Prussia birth records. (Source: https://www.ancestry.com/discoveryui-content/view/190129:61456?ssrc=pt&tid=178589381&pid=392328467116)

Here's the information by column -

Nro. / No. - 12 Note - this is a sunning number of all the births in the record - sometimes it is by year.

Männl[ich] , ehel[ich], unehel[ich], todgebo[ren] / Male, legitimate, illegitimate, stillborn - 5 Note - the number in the respective column is the running number for that category of birth, so in this case, it's the 5th legitimate male birth, probably for the year.

Weibll[ich] , ehel[ich], unehel[ich], todgebo[ren] / female, legitimate, illegitimate, stillborn - [blank]

Getaufte 1814 / Baptized 1814 -

Samuel, nat. den fünfzehnten May, r[enata] den 17 ej[usdem] - born 15 May, baptized 17 of the same month

Pater / Father - Michael Werner Instmann / farmer

Mater / Mother - Elenora gebohr[ene] [=nee] Streichin Note - the Surname is Streich, the -in is added to denote a female

Patrini / Godparents - Ferdinand Gens, David Schmuck [Knecht? can't tell because it's cut off = servant or hired hand], Anna Dor[othea] Schmuck, Regina Ferth, Mägde [maids]

Thanks for this - I have other female relatives who I've seen recorded with an "-in" last name when other relatives were recorded without that part of the last name. Wasn't sure what was going on there.

Under the godparents, I'm wondering about Ferdinand Gens. I see other Latin words in the birth record - do you think "Gens" here is an actual surname, or is this an indication that Ferdinand is a family member?

I think Gens is the surname in this case, but I'm not at all sure it's the entire surname, because it kind of disappears in the gutter of the tight binding. You could look for other birth records in this family and see if they used the same godparents, then confirm the name that way. Or look for other birth records if Ferdinand had a child.
