Could anyone please m let me know what this document is called? Is it a census. Thank you x

I don't think it's a census. If you look at the bottom of the second image, you'll see printed "Polizei-Verwaltung Itzchor." This leads me to believe that it's some sort of resident registration form for that city. Also, at the bottom of the first image, there are three sections, each with Zu- und Abzugsdaten and Wohnungen. This means Date Moved to and from, and residence. This gives you a sequential list of all the places this person/family has been. So in 1919, it says he was in the military, in 1922 he went to live with his wife, and in 1947 he died.
