I am in the process of creating this research matrix to various US states and territories.

I wonder what links you could suggest for Puerto Rico?

the Vital Records Online is a reputable company I think
but they are a private company separate and apart from the Health Dept of each state
as far as prices you are almost always better off requesting directly from the state and avoiding private companies - and you will get a lower price.

In the case of obtaining records for family born in Puerto Rico, VitalChek offers contact and guidance through the process by phone; in contrast contacting government offices tends to work when one goes in person.

agreed
I usually just mail in a ( filled out printed form from on line) and send directly by postal mail to the health dept of each state and get back what I want - with no need for phone assistance.
