Translation help for French Emigration passport
The Jewish Genealogy library in Cincinnati sent me what could be the emigration passport for my ancestor, Samuel Levy. I can't read it at all, and am hoping someone could translate it for me. Thanks so much!

Do you know more about this person. The name of his wife by example ?
