Where to find a readable copy

Below is a copy of a Puerto Rican death register from 1991. Does anyone know where I might find a more readable copy? I would like to know what the headings are, but this copy is too blurred when made larger. I also have looked in the same register for a better copy but can't find one.

The FamilySearch Research Wiki for Puerto Rico has a number of sample records from the Puerto Rico Civil Registry, perhaps you may be able to locate a better image of a record similar to the one you are trying to read. This is the link to the page:
