All I can make out is the name "Jovito".

Not sure how you didn't get an answer to this already. Here are the relevant bits:

• Record n°599;
• Registration date: 1916-06-29 @ 10:00 in Santa Isabel, PUERTO RICO;
• Child: Jorge Aponte;
• Born: 1916-04-31 [sic] @ 22:00;
• Race: mixed;
• Declarant: Jovito Carattini, divorced, businessman, born in Salinas [PUERTO RICO], resident in Barrio Jauca I of Santa Isabel, father of the child;
• Mother: Manuela Aponte, of legal age, single, works in the home, born in Santa Isabel, resident in Barrio Jauca I of Santa Isabel;
• Maternal grandmother: Eulogia Aponte, of legal age, single, works in the home, resident in Barrio Jauca I of Santa Isabel.

What doesn't make sense to me about this record is if he was the father and declared the child, why didn't he take his surname as well?
