Hints cannot be attached after editing the indexed details.
Today every time I follow a hint to edit the indexed details, it no longer allows me to attach the source. I get an error message "this record has been removed," but I can still search and find it in the results. Then if I click the camera I can see the image, but if I click the notepad icon I get the "removed" error. At first I thought it was just one record set, but it happened with two Philadelphia death records and a New York passenger list. No such problems on unedited hints. Expand post to see three images.

Thanks for sharing your problem. Sounds like there is a bug in the system. Did you provide feedback to FamilySearch.

This record shows in FamilySearch "Search Historical Records" index but when you click on it you get the message "Record Removed". I am thinking any edited records will be removed until administrators are able to double check it and hopefully will re enter it once it has been checked and approved.
I looked for an explanation in the Help Centre, to no avail. I would think perhaps you could send a message (through the Help Centre) to ask about it. The programmers would be the best ones to answer this. I would love to know the answer.

Editing has been in place for months and there's never been oversight. I doubt they would have changed that policy; if they had the resources to spare there wouldn't be so many errors in the published indexes. (I'm also an indexing reviewer, pre-publication.) The editing tool is there for contributors to take advantage of freely. Or it was until yesterday.
