First part is for your details. Гр. = Gr. i.e. громадянин = citizen (you put your name in there) Address below is yours as well

After the "statement" part you fill the details of whose birth record you are looking for. Again the name goes into гр. Rest of it is pretty clear, please let me know if you have any questions. The bottom part should have your passport number and date.

You may want to write the details of the person you are looking for in Cyrillic to avoid any translation confusion, but use Ukrainian spelling where possible because Ukrainian archives understandably may be very sensitive about anything written to them in Russian right now. So for example if you need birth record for Andrew Kozlovskiy, write "Андрій Козловський" rather than "Андрей Козловский" even if he was referred to like this in other Russian empire era documents. I may be making this all up, but at least that's what I would do. Maybe using English is even easier and more straightforward option.
