Hello Steve,
This is a record of a Jewish marriage. For such, special conditions applied in Bohemia under Austrian rule in the 19th century, see https://en.wikipedia.org/wiki/Familiants_Law. In particular, the total number of Jewish marriages were limited and required a special permit (Eheconsens).
Column 4 refers to that permit. The first of the subcolumns states that the permit applies to the city of Ledeč. The third subcolumn states that the permit concerns the operation of a tanner's business (for the bridegroom).
Column 5 names the groom and his parents: Joseph Moravec, third son of Moyses Moravec, "Schutzjude" (protected jew) of Ledeč dominion, residing in the town of Ledeč N:C: 186, and mother Maria née Nettel from H__?, Königgrätz county.
Column 6 names the bride and her parents: Anna Meiszl, legitimate daughter of Joachim Meißl, "Familianten" (permitted jewish resident) in Faudleb(?) in the dominion of the same name in Königgrätz county, resident in Věž N:C: 22, and the mother Sara née Reichmann from Petrkau on the dominion Schret__? in Caslau county.

Column 10 names the rabbi who performed the marriage ceremony: Lewi Mautner, "Religionsweiser" (religious wise man?) in Koschetic(?) N:C:
Column 11 names the witnesses: Joachim Rosenbaum, merchant in Ledeč N:C: 186; Hermann Meißl, merchant from __ ___ Königgrätz county N:C: 46
Comment: I am unsure of many place names because they are unfamiliar to me.
