German- Czech death record
The record in question is on the right side, third down for Adolf Morawetz. First, the writing is very poor, but I would appreciate confirmation of what I think I can read and any additional information you can pick out of this. Here is what I think I can make out:
- Adolf Morawetz
- Died: 4 Dec 1917
- Burial date not shown
- Father: Josef Morawetz
- Mother: Anna Meisel... more is said there but I can't make it out
- Age: 64
- Birth date: 22 Jan 1853.. does that possibly say he was born in Ledec
- Cause of death: Cerebral apoplexy... more is said but not sure what it is
Adolf lived in Haida; there is a town Zittau not far. Is that being mentioned in some way on that last line?
The date 4 Mar or May 1856 appears after the mother's information. Can you tell what that is all about?

Here's my additions to what you've already deciphered -
- Adolf Morawetz - [some sort of occupation after his name, but can't make it out] in Heide N. 100, Bezirk Heide, polit. Bezirk Böhm. [Liege?]
- Died: 4 Dec 1917, 12:30 a.m.
- Burial date not shown
- House number 100
- legitimate son of Josef Morawetz, Lohgerber [tanner] in [Ledetsch?] N. 137, Gerichts und polit. Bezirk [Ledetsch?]
- Mother: Anna legitimate daughter of Joachim Meisel from [Vez?] Bezirk [Deutsch-?-], verehelicht [-?-] 7 Mai 1856
- Age: 64 10/12
- Birth date: 22 Jan 1853 in [Ledetsch?] 137
- Cause of death: Cerebral apoplexy laut Totenb[-?-] Zettel ddto 5 Dezember 1917, N. 119 = according to the death certificate #119, dated 4 Dec 1917
The two lines underneath that 1856 date read: "Nicht versehen, weil ganz plötzlich vertorben. Wurde nach Zitten überführt und im Crematorium eingeäschert." Which means: "Not provided*, because he died very suddenly. Was brought to Zittern and cremated there."
*The phrase "nicht versehen" appears several times on the same page, and the context does not lend itself to "provided" as the right translation - maybe wasn't given last rights?
