In Full Name Review. Do we add the last name if it isn't right next to the first name?

Example: Fred and Sarah Johnson. [Only Fred is highlighted]

Does he become: First Name : Fred. Surname: Johnson

Or does he just get listed as: First Name Fred? [with no surname, because "and Sarah" is preventing the surname from being next to his first name.
