The remote access Record Lookup Service is no longer available. It was offered during the time that the FHL and FamilySearch Centers were closed due to the pandemic. Once centers reopened, it was discontinued.
The form only says discontinued on 17 July, without a year, but it ended in July 2023.
If there is a specific record you need, someone who visits an Affiliate Library or FamilySearch Center may volunteer to help.
