Spouse last name - US Ohio naturalization

Indexing US Ohio naturalization papers.

If the spouse's last name is entered as her married name (not her maiden name), should I enter the married name or mark the field Blank?

    If it is listed as her married name; ie Jane Doe, then Doe is entered as the surname.

    If her surname is listed as any other surname; Jane Jones, then Jones is entered.

    If there is no surname; Jane as the GN, then the surname field is marked blank.
