I have a document, it has been signed by the same person as on image 1.

Bottom of the document 2 states Police Court For The District of Newburyport,

Sworn to be petitioner, before said court etc.

What type of form is it?

    Could you post the batch number? The batch number is located at the top of the indexing screen and looks like [MQKV-2JP]. It is also located under Batch —> About Batch and Help —> Share Batch.
