How do I get fields back to their original position? Mine got mixed up.

I don't know if there is a "Reset to defaults" option, but you can click and drag the fields into your preferred order.

I just checked, and there IS a reset button. On the blue bar above the entry fields, there are three dots to the right. Click those, and it will bring up an options menu. The bottom button is "Reset Field Order", which does exactly what you want.
