Translate church death record

Can anyone please help with translation of my ancestor Adam Anstadt's church death record? He's #34 on this page. Specifically, the two far right column categories/titles at top of the page and the information contained within them for my ancestor. Thank you very much.

I suggest trying the Germany Research group for this, because I can't even quite figure out the column headings, nevermind their contents. (Best guesses: Adam Anstadt, died 4th, buried 7th June 1857, parents blank, age, illness: in 30th ??, ??. I've never encountered age and cause of death combined in a single column, though, so I'm doubting my interpretation of the heading: "Alter, Krankh.")
