I have a birth record that has a comma between two names on the same line. Is this for twins or for a 'middle, first' name?

I've seen commas between multiple given names of the same person, but I've never seen it to indicate reversing the order. I'd index it as "Kirsten Rosemary" and leave figuring out the reason for the comma to the researcher who finds the record.
