Do I index the following when its looks like she no longer lives there.
How do I index the following: I am not sure which one died.

Yes, you index Rebecca. You index John and put Mary in the spouse's name fields. It doesn't matter that one of them died. We index those residents listed, whether they've moved or died or not.
