Translation request- marriage record
The index indicated this is the marriage record of Vaclav Veverka and Anna Zalesky. I can see some of it but there are so many notes here- I'm just guessing. It seems they might be indicating a remarriage as well as birth dates. I could really use help on parents names too. Can you help me please?

Notes: Otec nezletilé nevěsty Anny Zaleskej k s..tka toho S[vatého] manželstva swoleni dává a wlastnoručeně podpisuje: Matěj Zalesky, podepsal (= father's approval on behalf of her non-adult daughter)
There are two witnesses mentioned on the right (Jan and Václav) and another below the note, Karel (Sunkár ?) soused a kostelnik z Lhoty.
