Arizona Birth Index Affidavits
In the Arizona, Birth Certificates and Indexes, 1855-1930 collection, some of the records have a reference line such as "Aff. Vol. #3". For an example see this record.
I think "Aff." stand for "Affidavits" - is that correct? Does this mean that there's a book of affidavits somewhere, and if so - where?
In such cases, I usually can't locate an actual birth certificate. Just the index card.

@mattjohnsonpint 1903 is before the Federal law mandating that births be recorded. There are a few that were recorded by the county, but this George Hornbeck that you have used as an example is not listed in them. My guess is that this is a delayed registration of birth, perhaps not even filed as a birth record, but as some sort of proof of his age so that he could register for Social Security. I would agree that the reference is to a volume of affidavits. I don't see that FamilySearch has any of the affidavit books listed in the Catalog, so I'd recommend calling down to Maricopa County Recorder (or archive) to ask about them.
