please check it for accuracies? Image 2 was rather different. It had affidavits at the top and at the bottom with a request from the petitioner in the middle.

    The first section is an older type of an Affidavit of Witness. The last two parts are an older OOA.

    Image 2 will be marked NNED, as it is summoning John Johnson to first become a witness, then his Affidavit of Witness, which is not indexed.
