How should I go about indexing this document?

I began indexing this document today, and both photos are of the same document, the only difference being a small sheet of paper flipped on either side in the middle. Should I only index the page that is exposed totally on either image? Should I index all of the exposed entries even if I repeat?

That is an overlay. You don't index any of the information on the page that is under the overlay. Without being able to see the batch, or the reference images, I think you should index the right side of that image though.

To share a batch so that others can see the document you are seeing, you need to provide either a link or the batch code. The batch code is the series of letters and numbers in brackets after the title. [XXXX-XXX]
