Since there is no example to go by for this type of Intention, should I index it as Middle District, Pennsylvania or Scranton, Middle District, Pennsylvania? I'm leaning towards Middle District, but want to get a second opinion.

I vote for Middle District, Scranton, Pennsylvania since it is the Middle District Court that operates from Scranton, PA. Actually, I would only index Scranton, Pennsylvania since the Middle District really isn't a "standardized place". It is curious that they are using middle district in the example.
