Do I leave the sir name of the baptized person blank record it as the same as his fathers sir name?
Also Is the name in parentheses after "late" the mother's maiden name?

I checked a few names indexed by Ancestry and found the child surname matched the father : example of child John Blain; father Joseph Blain; mother surname indexed as Molineux Blain --- this follows FS General Indexing Guidelines in years past ---and yes, the name in parentheses after "late" in the mother's maiden name --- from https://www.archives.norfolk.gov.uk/help-with-your-research/family-history/parishes-and-churches/parish-registers/information-given-in-parish-registers I found lots of helpful information about old English/Great Britain/United Kingdom Parish Registers.

Thank you, for your help. I have much to learn. I'll have to check into checking into the Parish Registers.
So I understand that the maiden name will be indexed as the mother's surname.

You leave the child's surname blank in this situation, just as they have in the project instruction's example. You could use the husband's surname following the mother's maiden because it has (late). However, based on the field helps, If not for that word late being included, you would not assume the husband's surname with the wife.
You can only use information from outside sources (Ancestry/Parish registers, etc.) to verify what you are seeing in your batch, but, you can't use that additional information on an index.
