For the name of the Father and Mother there is listed the name of an orphanage. I guess I will leave the name of the Father and Mother blank in this case, but I want to be sure.

The parents names should be blank. They are either dead, missing, or gave the child up for some reason. Plus, you wouldn't want to link an orphanage to a family tree.
