Difficulty Submitting a FHC Monthly Report

Kia ora, I am having difficulty submitting my monthly FHC report to HQ in Salt Lake. In the past I would merely google "Monthly FHC Report" and it would pop up , then it was plain sailing after that to complete the required details.

We have been in lockdown for a time and are now opening up again to working with patrons in the FHC and in their homes and I wish to submit a report . However the "google method" doesn't work any more.

I booked an appointment with a consultant to assist me online and he encouraged me type in "fhcprofile.familysearchsupport.org" which I did, and then it asked me to enter my email address and password , which I did, but it wouldn't accept it. Dilemma.

I am hoping someone can assist me.
