Is there an explanation for a Social Security form ?

Have ordered and just received the Social Security form "Request for E/R Action" filed in 1966. I ordered it in hopes of finding clues to follow in researching this person. The problem is that many fields do not clearly indicate what was being asked for in that field and some comments are cryptic. Some are easier to figure out F N is father's name M N is mother's name. Underneath the name of father is a field hard to read, may be P B. Would this be the place of birth for the applicant or the father? Also the remarks are unusual . Spaced out are MI Y MRN KURISU

Any help would be appreciated

    The FamilySearch Wiki has a list of the information you would expect to find on the Social Security form. I imagine the remarks are internal and you won't find an explanation. https://www.familysearch.org/wiki/en/United_States_Social_Security_Administration_Records
