Here is a translation for you:
entry 13 Chrustowice
In the town of Opatowiec, on the seventeenth of February/first of March 1886, at 9 o'clock in the morning, Jan Bartacek, forty-seven years old and Wikentij (unsure on this surname. He is just a wittness, so not as important. Let me know if you absolutely need it), forty two years old, both peasant farmers and residents of Chrustowice, appeared and testified that in the same village [Chrustowice], yesterday, at 10 o'clock in the morning, Tekla Poltorak nee Wistkow died at seventy years old. She was a peasant female worker/laborer and daughter of Benedykt (? - this name is a little bit unclear, but my best guess is Benedykt. If you are able to find her birth record, I can let you know for sure ) and Elzbieta Wistkow, and is leaving behind four sons and two daughters. After the death of Tekla Poltorak was confirmed, this act was read and signed by the author on behalf of the witnesses.
Signature of the registrar (Ignati something...)
