This is a birth certificate, but with a twist.
Parish office Alservorstadt Wien 8 (the address of the parish) in 1907, number I-1213, Johann Josef Lenz on 8 February 1907 was born in Wien 9 Alserstrasse 4 (address of the mother). Mother: Juliana Lenz, Roman Catholic.
Change of registration: The surname mentioned by decree on 12 October 1914 was changed to Soderberg.
This certificate was made on 21 October 1971 and signed by the parish priest.
I am pretty sure that this is what the change of registration part means, even though there are a lot of abbreviations. If anybody has anything to correct or add, you are welcome. This can't be the original birth certificate, but another one that has been changed because the handwriting is the same throughout the record and the dates are so varied.
