I would not say Kristen Nielsen Kristensen Ravnborg ever changed his name, the Ravnborg exists in previous generations and both Kristensen and Ravnborg existed at the same time.
I don't know the meaning of Fik preceding the date 1906 March 5th
It seems like maybe someone is looking at Niels Christian Ravnborg, the one born 1880, LQYB-538
That Niels is at Nykøbing Mors in the 1906 census and perhaps someone is looking to affirm his birth/christening but I can't decide whether they incorrectly think Kristen Nielsen 1871 LH6H-TTP is the Niels Christian in Nykøbing Mors or they are noting that this is not him.
There are similar notations on other individuals on this page so it's interesting.

Translation: Received 5 March 1906 name certification from the city and district bailiff in Nykøbing Mors for the name Kristen Nielsen Ravnborg.
The way I understand this is that civil authories in Nykøbing Mors authenticated or certified the use of the name Kristen Niesen Ravnborg for Kristen Nielsen Christensen.

I got some help from one of my FaceBook genealogy groups. Here is an answer as to what this is all about...
In 1904 a new law went into effect. It allowed people to change their family name once (for a modest fee). This would be recorded next to the original baptism in the church book and with a reference number. I have several examples in my family and most are from around that time. Since many people had a very common -sen name, quite a few used this opportunity to change their name.
So, as J Martin posted, Kristen Nielsen did officially change his "legal" name to Ravnborg instead of Christensen
