Requesting Translation Assistance

Hello,

I just posted a similar discussion for a different record. This is not a duplication. I would like to request translation assistance for a birth record that I found, possibly for a previously unknown relative. It looks as though the surname is Piasecki/Piasecka, but I am not certain of this. The record is located at the following site:

The year is 1888. The record is on slide 27. The record number is 95.
