Birth Record No. 83, Pamięcin Parish, Kalisz, Wielkopolska, Poland, of Stefania Sipka, born in village Poklęków on 15th November 1916, at 9 am, christened on 19th November 1916, at 9 am, daughter of Maksymilian Sipka, a farmer living in village Poklęków, 38 years old, and of his wife, Pelagia, born Kordylas, 30 years old. Witnesses: Adam Kuźniacki, 42 years old, and Józef Pawlik, 40 years old, both farmers from village Poklęków. Godparents: Ludwik Sipka and Stefania Kuźniacka.
