    Birth Record No. 14, Blizanów, Kalisz, Wielkopolskie, of Wojciech Kucharzak, born in Biskupice, House No. 13, on 28th March 1827, at 10 am, christened on 1st April 1827, at 9 am, son of Walenty Kucharzak, a peasant, tenant farmer from village Biskupice, 38 years old, and of his wife, Anna, born Bartoszek, 26 years old. Witnesses: Józef Melka, 37 years old, and Piotr Melka, 40 years old, both peasants, tenant farmers from village Biskupice. Godparents: the above-mentioned Józef Melka, married, and Marianna Graczowa, married, both from village Biskupie.
