surname was continued from previous page but don't know what it is.

The surname was continued from previous page but it's not included in this batch. Do I leave the surname blank and index the other information or skip those without a surname listed and start the record with the first surname?

    You will use the last button on the vertical toolbar. This brings up References Images. You can go back five and ahead five. Start with the -1 Reference Image and work you way back, if necessary.
